Step by Step Instructions:
From the Presentations tab click on the 'Tools' tab and select the 'Forms' option
2. Click the 'Create Form' button
3. Assign a Name for this Form, and click the 'Create and Configure Form' button to confirm. This name will only be viewable by admin users
4. Click the 'Add Field' button to choose from a menu of question formats
5. Select the Field format that’s most suitable for your intended question. The 'Next' button will become clickable once a Field is selected
